Job description

We're hiring a Service Facilitator to support individuals in their recovery and help them achieve stability and independence through service facilitation. This role collaborates with clients, their families, and healthcare teams to create and implement personalized recovery plans. This role creates impact by providing in-home and office-based support to older adults and persons with disabilities in Madison, WI, with some remote work possible after demonstrating job competency. Work is performed in a hybrid model. Key Responsibilities: - Provide support for individuals working toward recovery using a supportive, strengths-based, and culturally humble approach to treatment. - Facilitate and coordinate services relevant to individuals' recovery plans and community support needs using trauma-informed and non-stigmatizing strategies. - Assist program participants in identifying and establishing natural and professional supports in the community while monitoring progress and engagement.

Job benefits

Job requirements

- Six months experience working with individuals with mental health conditions, disabilities, chronic illnesses, or seniors. - Valid driver's license, reliable transportation, and current auto insurance for travel in Dane County, WI. - Ability to work in environments with potential hazards such as odors, second-hand smoke, or adverse living conditions.
